Spontaneous Remission of Primary Central Nervous System Lymphoma

Abstract

|
We report a rare case of primary central nervous system lymphoma(PCNSL) that regressed spontaneously in serial brain magnetic resonance images(MRI) without specific treatment. A 79-year-old woman was admitted with a mass lesion in the left frontal subcortex. Upon admission, she complained of mild right leg weakness. The patient was observed without any treatment because she refused further treatment. Two months later, the follow-up MRI showed that the enhanced lesion had disappeared, but a new lesion was detected on the splenium of the corpus callosum. This new lesion also disappeared in the follow-up MRI, but another new one was detected in the left basal ganglia. This lesion also disappeared in the next follow-up MRI, but the initial frontal subcortical lesion reappeared. Biopsy revealed that the lesion was a malignant lymphoma. Due to the fluctuations with improvement in both clinical course and radiological findings, alternative disorders were considered, which led to a delay in the diagnosis and treatment prior to the brain biopsy. This case shows that PCNSL cannot be reliably ruled out by improving or disappearing symptoms, signs, or traditional MRI abnormalities without biopsy.
